IYA Basketball Season Registration

Get ready for another exciting season of IYA Basketball! Open to youth Pre-K through 6th grade, this program is designed to build skills, teamwork, and confidence on and off the court.

Registration Fee: $75 per player

PAYMENT DUE MON, NOV 3RD
Included in Fee: Official basketball jersey and shorts
Bonus: FREE developmental/fundamentals basketball camp offered before the season begins.
Note: Teams may be combined depending on the number of players in each age group.

Parent/Guardian Consent & Waiver

I give permission for my child to participate in Itasca Youth Association (IYA) sports. I certify that my child is healthy and able to participate. I understand there are risks of injury in sports and related activities, and I accept full responsibility for these risks.

In case of injury, I authorize IYA to provide first aid, arrange transportation, and/or secure medical treatment by a licensed physician. I agree that all medical expenses are my responsibility.

I release IYA, its board, staff, and volunteers from any claims related to my child’s participation. I also agree to return or replace any equipment issued to my child. As a parent/guardian, I understand I may be asked to assist with program needs such as fundraising, coaching, or field prep.
